[ANSTHRLD] Broke website stuff

Website notes:

For those code people out there, you might be amused to know that when
Dreamhost upgraded our PHP version to something produced this century (read
7.1+, I forget exactly), it blew up a lot of old code we had.

For the non-code people out there: some stuff broke.

Reis, using his sneaky "I code things for Zodiacus/the OP" powers, has
fixed the main achievement armorial page with a bandaid fix that does at
least work. The rest of the achievement pages (ordinaries and indexes) are
not currently working. He does intend to fix them all properly later; this
fix was by necessity fast and ugly.

The achievement registration form probably still works. (A test mail I sent
with it did arrive.)

The quarterly reporting form does not work. This is not under the OP
section, and we're not sure who wrote it. Who gets to fix that is up to the
author and/or Star.
