IMO, either highest title, or most appropriate.  Title stacking looks and sounds "Just Silly."(TM)

I.e. a letter would start out

"To His Grace, Duke Kein McEwan, KCSA, OP, etc etc"

Not 
"To His Grace, Duke Sir Master Kein McEwan"
