Good old Bartleby, home of the wild and free public domain!  Fanny Farmer's 1918
edition is available online <www.bartleby.com/87/> which the Bartleby people chose
because it was the most recent edition actually edited by Farmer.  The
reproduction 1896 [still ed. by Farmer] seem to be readily available from modern
book dealers, and the 1884 pre-Farmer edition is still findable from antiquarian
book dealers.

Scanning Bartleby:  there is no entry for BOSTON CREAM PIE but there is one for
CREAM PIE under CAKES [http://www.bartleby.com/87/0031.html], with variations
following. Hey, they don't call it a New York steak in New York either.  Basic
recipe [remember, it's public domain]:

                                "Cream Pie I
                     1/3 cup butter
                                    1/2 cup milk
                     1 cup sugar
                                    1 3/4 cups flour
                     2 eggs
                                    1/2 teaspoon salt
                         21/2 teaspoons baking powder

 Mix as one Egg Cake. Bake in round layer cake pans. Put Cream Filling between
layers and  sprinkle top with powdered sugar."
