Hmm....I just have to try my hand at this. It's obviously a salad of some
kind.
   I would guess that the all-meat patties would be chicken, beef not
tasting that well on salads. I would guess that the reason that it's
mentioned as 'all-meat' patties is that there was widespread tampering and
selling of fake-meat products in this time period. I would grill the chicken
patties (breasts), them place them in a drying oven or under a hot lamp til
dry.
   I have done some research on the "special sauce", and what I came up with
was a concoction of an egg and oil dressing as a base, with diced
pickles(what kind wasn't stated, so I used a simple mix of carrot,
cauliflower and pepper pickle that I had lying around.), tomato paste and
salt, pepper and other spices. I am assuming that the pickle and onion
referred to are merely typos and meant to go in the sauce. I chopped the
onion fine as well.
   For the lettuce I used a mix of wild greens. I added chopped tomatoes to
the salad mix.
   I believe that all this was held in a small loaf of bread formed into a
bowl using a simple method of rolling out the dough thin and putting it on
an upside down flowerpot(clean please!) so that it will form a bowl when
cooked. I simply added sesame seeds to a regular bread recipe.
   I placed the greens and tomato mixture in the bread bowl after it had
cooled, chopped the chicken in cubes and added it to the salad, poured the
special sauce on top, and garnished with the carrot, cauliflower, pepper
pickle, tomato, and onion.
