Obviously, there is still some controversy on the matter of the anchovies.  Most
sources seem to imply that the dressing was made with the contents of a
restaurant's mostly-bare cupboard and improvised on the fly and anchovies were
only added later.  Here is a good description and recipe:

the part where you rub a garlic clove in the salad bowl before tearing and
tossing the lettuce.
