Certainly available in late period in Engand - search for recipes for "A dish of Snow".  It's not my time period of interest, so I don't have an actual link to it, but it's been discussed here many times.

To the best of my knowledge, no, although there are plenty of references to pasta with cheese on it - usually not cheese sauces, though.
