While I agree with item 4, I must respectfully disagree with #3.

The labor union that I am a member of regularly schedules moderated chats
with people logging on nationwide.

Having the moderator keep track of whose turn it is to bring up a new
subject (private message to the moderator initially) and being ready to go
when it is your turn (again this part is for a new subject) and allowing
anyone who wants to contribute to do so during the bulk of the  discussion
seems to work well for us.

This allows those who are interested to have direct input or at least see
what others are saying in a timely manner.
