I believe that the reason that this hasn't been implemented yet is because the fine points such as these are still being worked out. For most kids, the buddy system should work well for restroom needs, but, as a substitute teacher in grades from K-12, I can tell you that the group break is used often. The reason being is partially because if one gets to go, pretty soon they all want to go. It's easier to head 'em off at the pass, as it were. I see this used especially with the younger kids, but when I sub for in school suspension in the older grades, they use it, too, or have predetermined break times.
 
The whole idea of the parents remaining in the vicinity or helping is to avoid these difficulties. If you are holding an activity and have at least two parent or adult volunteers that are unrelated, you should be able to leave the kids with them and take Susie to the restroom if her mother isn't there. The two deep unrelated insures that you never leave one unbackgrounded adult with the kids. The likelihood of something happening with two unrelated adults and a bunch of kids is extremely unlikely. It is more likely for susie and her buddy to run into trouble on the way too, or in the restroom.
 
Children's activities should be held within sight of the nearest restroom and be within 100 feet of it, ideally.
