Or men.  Besiegers were camping more or less outdoors in an era
with slight notions of sanitation.  Eventually they'd be visited by
what a later age would caler Corporal Forbes (cholera morbus).
The people inside had permanent structures (modulo attackers with fire
arrows, catapults, or whatever as appropriate).

One of the reasons Henry II Plantagenet was a military genius was
because he was a notable taker of castles.  It *could* be done if you
had resources.
