Remember other standard sources, such as _Butler's_Lives_of_the_Saints_.
The EB is not the only "standard source" out there.

Those can, do, and have slipped through.  If they're caught, fine.  If
not, the world isn't going to come to a screetching halt.  :)  In the
dark, distant past of the SCA such things got through.  Now with online
searches we can catch them, but it's not such a problem as if Ewan Chormacc
were missed.

Again, I don't think it's an earth-stopping problem.  Even with the best
conflict checkers checking with the best tools, things get passed which
probably shouldn't have.  Last year a 1 CD difference was passed because
nobody caught the conflict (might've been in '99, was definitely after I
started commenting).  The CoA shrugged its collective shoulders and admitted
to being human.
As a matter of fact, last year two identical names were passed about four
months apart.  Identical.  Submitter #2 had to be asked what sort of
changes he would take.  This isn't the first time.  It happens.  I'm not
saying it's good, but I'm all for the human factor, even if the occasional
mistake occurs.

So my best advice is check the best you can during consulting, trust that
the CoH and CoA might catch anything you didn't (this is why we do this
sort of checks-and-balances submission handling in the first place), and
don't agonize over the result.
