"Tenterhooks"

A "tenter" is a stretching frame to size and regularize freshly-woven
woollen cloth.  A "tenterhook" is a small hook (they look a lot like bent
nails) closely and evenly spaced around the perimeter of the tenter to hold
the cloth under tension.

To "be on tenterhooks" would be quite painful, if it were literal rather
than figurative.  <-:
