A fantastic book for info on table settings is Boke of Keruynge,  Peter 
Brears (ed.), Southover Press.  ISBN: 1-870962-19-2
It goes into all sorts of detail on the placement of trenchers, bread, 
table linens, salt cellars, etc...

Way cool stuff!
